    OWNER-OPERATOR INFORMATION
    Line haul miles (loaded miles)
    Average Miles:  2600-3000 miles

                 All Dispatched Loaded miles        $0.90  PLUS FUEL SURCHARGE!!!            
                 All Dispatched Empty miles          $0.90  PLUS FUEL SURCHARGE!!!

    Fuel Surcharge:  Evaluated bi-monthly.
    Loading/Unloading Pay:  $50.00.  The Company will pay for approved lumpers.
    East Coast Pay:  $100.00 for delivery within the five boroughs of New York City-not forced dispatched.
    Toll Tickets:  Paid by the COMPANY when authorized.
    Base Plates and Permits:  Paid by the COMPANY.
    QualComm Satellite:  Installed at no cost.  $15.40 weekly cost to O/O
    Fuel Tax:  Paid quarterly by the COMPANY and charged back to the owner-operator.
    Escrow Accounts:  Mandatory Escrow Account:
                            $1,500 per unit.
                            $50.00 deducted per week until $1,500 is reached.
                            (Interest-earning account)
                    Maintenance Escrow Account:  (optional)
                            $0.03 - $0.10 per mile
                            (Interest-earning account)

    Standards:  All Owner-Operators must meet the same hiring standards as company drivers.
                    All owners must be the actual driver of the truck.

    OWNER-OPERATOR PROGRAM

    INSURANCE INFORMATION

    Bobtail:  Provide own or purchase through the COMPANY, coverage limit $1 million minimum.
    Physical Damage:  Optional, available through the COMPANY, cost based on value of truck.
    Occupational Accident:  Provide own coverage showing COMPANY as the certificate holder or purchase through the
                    COMPANY (available for owner-operator only).
    Cargo & Liability:  The COMPANY provides coverage under dispatch, at no charge to owner-operator.

    ORIENTATION

    Owner-Operator is required to bring truck to the COMPANY's General Office/Terminal in AR.  Orientation class is held each Tuesday and Friday.  
    Owner-Operator will be paid $120.00 on first settlement for completing orientation and reimbursed for truck fuel.

    Owner-Operator must bring the following to orientation, before a contract can be completed:
            
    •        Form 2290 (Highway Use Tax), stamped by IRS as paid, showing VIN of truck.
    •        Copy of Title (front and back) and lease agreement, if applicable.
    •        Registration (if title has been applied for and not yet received).
    •        Bill of Sale or security agreement.
    •        3 months prior maintenance records.

    Truck Requirements:
            
    •        Ten years or newer.
    •        Must pass DOT and COMPANY inspection.
    •        Wheelbase must be 260 inches or less from the center of the steer axle to the center of the tandems.
    •        Weight must be 19,000 lbs. or less, with half tank of fuel.
    •        Fifth wheel height must be 48” or less.
